Derrick is an Irish, recovering drug addict who is the brother of Patrick McReary, Francis McReary, Gerald McReary and Kate McReary. He went back to Ireland due to Aiden O'Malley snitching on him and other problems. He accompanies Niko, Packie and Michael on Three Leaf Clover where he blows the vault door open.

Francis McReary becomes paranoid when he hears Derrick is back in town and asks Niko to kill Derrick. Francis organises a meeting with Derrick but he asks Niko to protect him from Francis, as he suspects that at the meeting, Francis would kill him. Niko is placed in the position to choose to either kill Francis or Derrick.
If Niko shoots Derrick, he attends the funeral, but when it is attacked by Albanian mobsters and the police, Niko and Packie drive away, and the coffin begins to fall out. The player must drive to the funeral without losing Derrick.
