If you're having trouble with a girlfriend, ask for help here.
There are multiple girls you can meet throughout the game and become your girlfriend, not just the 2 you meet through normal progression of the game. Here is a list of the girls, times to meet them, where to meet them, and more. The information comes from the guide, I thought it would be helpful for those of you out there who have questions about girlfriends in the game.
Keycard Issues with Millie?
The simple answer to this is to kill her on a date and you will get the card. This will not affect your 100% stat. I put this at the top of the page since I know it's a frequently asked question.

Occupation: Cop
Location: Barbara is found in El Quebrados, Desert in the Sherrif's parking lot.
Taste: She likes you with muscles and a lot of sex appeal.
Favourite Restaurant - Jay's Diner in Tierra Robada
Availibility: 16:00 to 06:00

Taste: She likes you normal, no fat and not too muscular (20%).

Taste: Pose as a gimp and make sure you have a lot of sex appeal.

Clothes, haircuts, tattoos, muscles, and your cars all count towards your sex appeal. Clothes go to 60% of your total sex appeal and Vehicles go to 40%.
Make sure you don't damage your car on the way to your girlfriends house, it will deduct sex appeal points.
Bring Flowers to your girlfriend. On the date, surprise her by facing her and pressing L1, then press L1 again to kiss her after giving her the flowers. Each add 1% to the Girlfriend Progress slot.
Don't let your girlfriend take too much damage while driving on a date. If you do, you will lose her as a girlfriend and not be able to date her again.
Be quick on dates. Your girlfriend can get bored if you don't get to where she wants to go quick enough.
Do what she requests. When your girlfriend asks to do something, do it. If she wants to go get a drink, go to one of the Martini glasses on the map. Make sure you can dance good too, she might want to go to a club.

When you get your relationship to 100% with a girlfriend you get a costume delivered to your wardrobe.

Millie - Gimp Suit (actually get this in first date)
Each girlfriend also has a vehicle which you will get the keys to when you get enough progress in your relationship.
